Loteria Nacional España (Navidad 2015): El mayor premio es compartirlo / Fábrica de maniquies /


Justino es guardia de seguridad en una fábrica de maniquíes. Al trabajar en el turno de noche nunca coincide con sus colegas de trabajo, pero irónicamente pasa sus noches rodeado de figuras humanas. Su soledad y sus ganas de compartir le llevan primero a interactuar con los propios maniquíes, y más tarde a comunicarse a través de ellos con sus compañeros del turno de mañana. En su afán por hacer cosas por los demás, Justino se llegará a olvidar de sí mismo pero el destino querrá que sus compañeros no se olviden de él.

Mi ZADEC cumple 30 años...en Phillipines.


Ayer Sandoz, hoy Klinos
Era el último trimestre de 1983, entonces en Sandoz Venezuela.

Como Product Manager, desde su "lanzamiento", había sido responsable de Zaditen (Ketotifeno Sandoz) en ese país. El producto tenía un desarrollo exitoso ejemplo para otras afiliadas.
Al otro lado del mundo, en Phillipines, Wander  también lo había introducido pero con resultados no muy satisfactorios.. 
Se abrian dos opciones: relanzar o...empezar de nuevo. 
Contaba en ese país con una filial, Sandoz, escogida para la segunda de las opciones.

Desde la sede central en Basel (Suiza) decidieron por mi, Mktg. Services Manager en Venezuela. Sería el responsable. Todo iniciaría las primeras semanas de 1984.

Viajar de Caracas a Phillipines es largo viaje. Salir un viernes am (Miami, Los Angeles, Honolulu) y llegar un domingo am a Manila. Me esperaba Leo Estevez, Product Manager Sandoz, responsable local para el proyecto.

Fué una larga estancia/experiencia en ese país que implicó el desarrollo de todo un Plan de Introducción y Marketing  para un nuevo Ketotifeno (ahora Sandoz) al cual tuvimos que, entre otras múltiples actividades, poner nombre.



ZADEC fué el que escogimos.

Al regreso la ruta fué inversa (Bangkok, Tokyo, Dubai, Zurich) y llegar a Basel, mi primera vuelta al mundo", para presentar el proyecto. Fué aprobado.

ZADEC se lanza en Phillipines en 1985.

Mi vida laboral, desde ese momento, tuvo una gran movilidad. A poco de regresar a Caracas soy trasferido a Perú (1984 a mediados) como Jefe de Marketing.


También con Zaditen en nuestra cartera de productos de la mano de Carlos Anavitarte como Product Manager. Y, su hijo..., como "modelo" en nuestras campañas promocionales.

México luego (1987) como Gerente de Mktg. para regresar a Lima (1990) como Gerente de la División Farmacéutica y luego Presidente/Gerente General de Sandoz Perú.

Mis días en esa compañía terminarían (1998) en la filial española, a la que fuí trasladado en 1994, como Director Comercial.

Hoy, al cabo de 30 años, siento el orgullo de ver que "no aramos en el mar"...
ZADEC aun vive...

Con otra D.O./"paraguas" (Novartis) pero...vive.

Un gran amigo, Juán M. Feliz, en uno de sus viajes de trabajo a Manila, tuvo la gentileza de comprar en una farmacia los dos envases (bodegón) que encabezan esta "nota".

...lo ratifican. 
 
Hoy, antes de que finalice este año de su 30º aniversario quiero dejar constancia/historia de ello.

THE PIPELINE REPORT 2016 (III): Autoinmmune


Baricitinib Eli Lilly/Incyte 

Indication: RA (Ph.III) 
What the clinical trials found: The daily oral demonstrated superiority compared to placebo after 12 weeks based on ACR20 response (Ph. III RA-BEAM). The agent also proved superior to adalimumab on key secondary objectives of ACR20 response and improvement in DAS28-hsCRP score. A few occasional AEs were reported. 
Credit Suisse Success Probability and inThought Comment: 70%. The JAK inhibitor appears to have similar efficacy and safety to Pfizer’s Xeljanz. It was supposed to have a once daily vs. Xeljanz’s twice daily advantage, but Xeljanz’s once daily formulation will likely be approved soon. It’ll be interesting to see if Lilly/Incyte can do something with patient access and price to improve upon the poor performance of Xeljanz and expand the JAK inhibitor market. Expected launch: 2016 (Source: Credit Suisse) 
Credit Suisse forecast: $1.09 billion in global annual sales by 2020 
What the physicians are saying: Baricitinib is the most advanced competitor to first-in-class Xeljanz, which caters to patients who have failed anti-TNFs and/or methotrexate; an area where physicians have been awaiting more options. However, price could be a barrier to baricitinib uptake. With the biologic market’s expected growth by 2020, JAK inhibitors have room to develop in a market that seeks effective therapies to improve patient quality of life. —Anita Agier, head of Disease Atlas, GfK Healthcare 

Ozanimod Celgene 

Indication: Relapsing MS/ulcerative colitis (Ph.III) 
What the clinical trials found: Ozanimod in RMS reduced MRI brain lesion activity and met key secondary MRI-based endpoints (RADIANCE, Ph.II). Ozanimod in UC met all efficacy endpoints with statistical significance in patients on the 1mg dose after 32 weeks of treatment (TOUCHSTONE, Ph.II). No severe AEs observed to date. 
Credit Suisse Success Probability and inThought Comment: 25%. The drug looks similar to Gilenya in efficacy and perhaps better in safety. Surprisingly, Novartis never developed Gilenya in ulcerative colitis or Crohn’s disease, so ozanimod has a big advantage and will perhaps become an oral alternative to Humira and Entyvio. Expected launch: 2018 (Source: Credit Suisse) 
Credit Suisse revenue forecast: $900 million in annual global sales by 2020 
What the physicians are saying: Trial findings show ozanimod’s ability to reduce MS relapses and brain lesions. Early opinions indicate a similar safety profile to approved oral agents Tecfidera, Gilenya and Sanofi’s Aubagio, providing another oral option for needle-adverse patients. If results from ongoing Phase III studies show superiority to Biogen’s Avonex and similar or better safety to the current oral therapies, then neurologists could add a formidable weapon to the MS arsenal. —Paul Wojciak, research director, GfK

ABP 501 (adalimumab biosimilar) Amgen 

Indication: RA (Ph.III) 
What the clinical trials found: A Phase III trial in patients with moderate-to-severe RA met its primary endpoint of clinical equivalence between the adalimumab biosimilar and branded adalimumab groups, as measured by ACR20 assessment at week 24. The trial also met its key secondary endpoints, including ACR50, ACR70 and DAS 28-CRP. The safety profile was comparable to adalimumab. 
inThought Comment: There are at least 20 biosimilar adalimumab, etanercept and infliximab agents in global development, but the real question is how this will play out in major markets. We expect the market to support no more than four biosimilar versions of a given drug and believe that deals with payers will dictate winners and losers. 
What the physicians are saying: Despite concerns that these complex substitutes aren’t a perfect replacement, biosimilars will undoubtedly play a leading role in future RA treatment. Most physicians recognize that biosimilars lower costs and give access to a greater number of patients in need of more effective therapies. But a lack of product information and experience will impair their trust. Physician education is one of numerous hurdles the biosimilar will face upon launch as multiple other assets in late phase look to capture a piece of the Humira pie. —Anita Agier, head of Disease Atlas, GfK Healthcare

Merck KGaA & Merck conflicto de Merck_as / marcas?


Merck, a leading science and technology company, today announced the relaunch of its brand identity. The fundamental revision of the visual appearance as well as the introduction of a new logo reflect the transformation into a global science and technology company. At the same time, the brand architecture at business level has been simplified. Outside the United States and Canada, the company will operate uniformly as Merck

The investment in the Merck brand is part of the "Fit for 2018" strategic transformation and growth program, which includes the focus on innovative, technology-driven businesses as well as the modernization and expansion of global headquarters in Darmstadt, Germany. This also involves a more self-confident and at the same time clearer tone of voice, reflecting Merck's character and linking its pride in a nearly 350-year-old culture with scientific curiosity and a passion for research. 

With the introduction of the new brand design, the previously independent divisional brands Merck Serono and Merck Millipore will be eliminated. In the future, Merck Serono will operate as the biopharmaceutical business of Merck, and Merck Millipore as the life science business of Merck. "Following the two major acquisitions, the Merck Serono and Merck Millipore brands helped us to position the duality of existing and acquired businesses in the marketplace. We succeeded in doing so. That's why we are returning to the brand that we have been known under for nearly 350 years," Kley added. Product brands such as Erbitux or MilliQ will not be affected by this change. 

Merck holds the rights to the Merck name and brand globally. The only exceptions are the United States and Canada. The company will therefore continue to operate in these two countries as EMD Serono in the biopharmaceutical business, as EMD Performance Materials in the high-tech materials business, as well as EMD Millipore in the life science business up until the planned acquisition of Sigma-Aldrich has been completed. 

Merck's visual image is fundamentally new. It is based on the concept of a vibrant science and technology company, and is inspired by the colourful and multifaceted shapes seen under a microscope. The new Merck design features expressive colors and shapes with a scientific look-and-feel. These are also reflected by the new company logo. In comparison with the former logo, it makes a bold statement; its design is less complex and can be used in different colors.(Más)

THE PIPELINE REPORT 2016 (II): Oncology

Atezolizumab Roche 

Indication: Bladder; breast; renal; NSCLC (Ph.III) 
What the clinical trials found: A Phase-II study (POPLAR) met its primary endpoint and showed a statistically significant survival benefit compared to chemotherapy (HR = 0.54; p = 0.014) in people with recurrent NSCLC whose tumors expressed medium and high levels of PD-L1, which corresponded with people living 7.7 months longer than people who received docetaxel chemotherapy. AEs were consistent with previous studies. 
Credit Suisse Success Probability: 75%. Expected launch: 2016 (Source: Credit Suisse) 
Credit Suisse forecast: $4.45 billion in global sales by 2020 
What the physicians are saying: While a latecomer to the immunotherapy party, atezolizumab represents a new modification of current immunotherapies in that it targets PD-L1 (rather than PD-1), which is hypothesized to have less off-target toxicity. Urothelial bladder cancer (UBC), in particular, has not seen significant development in decades so any new treatment modalities would be welcome for patients and physicians. A biomarker driven approach in tumors such as NSCLC is well established and PD-L1 testing might be able to smoothly integrate. —Alex Bastian, VP, GfK Health 

Abemaciclib Eli Lilly 

Indication: NSCLC; breast cancer (Ph.III) 
What the clinical trials found: A Phase-I trial of abemaciclib found a 49% disease control rate for the 57 evaluable patients with NSCLC, including 2 partial responses and 26 patients with stable disease. The disease control rate for the KRAS-mutant patients was 55% vs. 38% for the KRAS wild-type patients. Leukopenia and neutropenia were the most common grade 3 AEs reported. No grade 4 AEs occurred. 
Credit Suisse Success Probability: 65%. Expected launch: 2018 (Source: Credit Suisse) 
Credit Suisse forecast: $550 million in global sales by 2020 
What the physicians are saying: Abemaciclib is the only CDK inhibitor that has shown promise as a single agent, leading to Breakthrough Therapy designation (in refractory HR+ advanced or metastatic breast cancer). On a pharmacologic basis, this could be best in class. Pfizer's Ibrance (palbociclib) has helped to introduce the drug class into the treatment paradigm. Most patients will be treated with a CDK inhibitor. Other drugs in the class have a week off due to AEs (neutropenias). With abemaciclib, you wouldn't need to stop treatment. —Alex Bastian, VP, GfK Health 

 ABP-215 (bevacizumab biosimilar) Amgen/Allergan 

Indication: Advanced NSCLC (Ph.III) 
What the clinical trials found: In a Phase-III study, the primary endpoint, overall response rates (ORR), was within the pre-specified margin for ABP-215 compared to bevacizumab, showing clinical equivalence. The secondary endpoints, risk difference of ORR, duration of response and progression-free survival were consistent with the primary endpoints. Safety and immunogenicity of ABP 215 were comparable to bevacizumab. What the analysts are saying: Branded Avastin (bevacizumab) had sales of $3.3 billion in 1H15, representing a huge opportunity for a biosimilar copy. This agent will provide an alternative for the cost-conscious US physician and payer community. Easier access to bevacizumab could be a welcome addition but this depends on how payers integrate biosimilars into formularies and pathways. Another unanswered question: whether this agent can get all Avastin indications. —Alex Bastian, VP, GfK Health

THE PIPELINE REPORT 2016 (I)

The most-promising late-stage pipeline drugs are a varied bunch: cancer therapies employing the immune system, PCSK9 inhibitors targeting cholesterol and biosimilars angling to upset biologics' market share. Some will hit the market with a splash, others will crash and burn due to safety/efficacy issues and the rest could fade into obscurity if competitors win the race to market. 

When judging the future of clinical candidates, the only important criterion is whether they will change therapy when approved, notes Bernard Munos, senior fellow at FasterCures. Of a potential drug, Munos asks, “Will patients and physicians clamor for it, as they clamor for [Gilead's hep.-C pill] Sovaldi or some of the new cancer drugs?” 

These days, changing the standard of care comes with a hefty price tag: The year 2015 saw more industry pricing controversy than ever before—and with the arrival of biosimilars prices may go haywire. 

Nonetheless, oncology boasts the most noteworthy R&D innovation of late. “PD-1/L1 inhibitors are the most remarkable accomplishment in years,” says Richard Evans, founder of SSR Health. (Más)


Palabras: "Wicked problems"


Un "problema retorcido" (en inglés, "wicked problem") es un concepto utilizado en planificación social para describir un problema que es difícil o imposible de resolver dado que presenta requisitos incompletos, contradictorios y cambiantes que generalmente son difíciles de reconocer. El término "retorcido" no se utiliza en un sentido de malvado, sino antes bien como resistencia a la solución. Además, dada la existencia de complejas interdependencias en este tipo de problemas, los esfuerzos para resolver un aspecto de un problema retorcido podría revelar o crear nuevos problemas. 

C. West Churchman introdujo el concepto de problemas retorcidos en un "Guest Editorial" de la publicación Management Science (vol. 14, núm. 4, diciembre de 1967) al hacer referencia a "un seminario reciente" de Horst Rittel, discutiendo la responsabilidad moral de la investigación de operaciones "de informar al mánager en la medida en que nuestras "soluciones" han fallado en dominar los problemas retorcidos". Horst Rittel y Melvin M. Webber describieron formalmente el concepto de un problema retorcido en un tratado publicado en 1973, contrastando los problemas "retorcidos" frente a problemas relativamente "controlables" y solucionables que se pueden encontrar en las matemáticas, el ajedrez o la resolución de puzles. The most authoritative and oft-cited view on how to approach wicked problems is in the work of Roberts, who suggested three possible routes. Described in the context of our market-led/ product-led problem, these are: (Ver)

  • -Revert to authority                                                                This approach would reduce the complexity of the question by reducing the number of people involved. In our example, a small, senior team would call the product development shots. One can immediately see the attraction of this streamlined approach but of course it depends on the small team having an excellent appreciation of a complex issue, which is not always the case. 
  • -Competing arguments                                                            This approach depends on encouraging the development of alternatives and enabling their competition against each other. In our example, competing productled and market-led initiatives would make their case. The merits of this approach - clarity and constructive confl ict - are attractive. However, it creates adversarial relationships and depends on an objective judging process, which is asking a lot of human beings. 
  • -Collaborative consensus                                                           This approach depends on enabling all stakeholders to share their thoughts constructively and move towards common agreement. In our example, technological innovation and customer needs would converge on an optimal product strategy. This approach sounds ideal in theory but is very time consuming, requires expert facilitation and is prone to infl uence by strong, not necessarily well-informed, egos. (Más)

Infidelidad: No me acuerdo quien lo había dicho pero se que lo dijeron...



Un estudio publicado en «Science» confirma que los ratones de campo que deambulan buscando nuevas compañeras tienen menos memoria espacial que los machos fieles.

Los investigadores de la Universidad de Texas en Austin y la de Cornell en Ithaca (Nueva York) sugieren que la promiscuidad que exhiben algunos de estos roedores podría estar relacionada con la expresión de «avpr1a», un gen que predice la fidelidad sexual en los machos de los topillos y que se sabe que tiene un papel clave en el circuito de la memoria espacial. Pfizer & Allergan: Sexo oral empresarial


La primera farmacéutica une Pfizer y Allergan, fabricantes de Viagra y Botox, lo que confirma, como viene avanzando la pornografía desde hace décadas, el triunfo del sexo oral. Las multinacionales de la turgencia del pene y de los labios exhiben su unión ante el mundo. Tienen otros productos en su catálogo, pero son conocidas y grandes por estos dos, relacionados con lo morboso en el sentido menos literal de enfermizo. La aplicación de esos productos da más morbo del que quita. 

Las dos empresas, llamadas globales, son estadounidenses y consuman en Irlanda esta práctica llamada francesa. Lo que excitó a las multinacionales para acoplarse fue la bajada fiscal, el punto G del neoliberalismo hallado en los ochenta en las relaciones transoceánicas de Margaret Thatcher y Ronald Reagan, de inicio de la revolución sexual de la Economía promiscua, insaciable y desprejuiciada respecto a los orificios practicables. Irlanda excita a las multinacionales por su alta producción de baja fiscalidad en un entorno seguro, anglófono, occidental y cristiano. 

El resultado de esta felación empresarial es un valor de 272.000 millones de euros, 110.000 empleados y una facturación anual de 60.000 millones. Los tres años siguientes no necesitarán vender muchas más píldoras azules ni toxina botulínica de tipo A para uso estético porque ya han calculado una reducción de 2.000 millones de dólares de su gasto base. Una buena cantidad que se quedan ellos y detraen a la comunidad. Como hacen negocio con el conjunto de los estadounidenses dejando de pagar unos servicios que seguirán utilizando, en la carrera a las primarias estadounidenses se escandalizan incluso patriotas antifiscales como Donald Trump, que califica la fusión oral de antipatriótica. En Irlanda puede que reduzcan empleo. Pero eso son cosas de gente. Y la gente les hace lo mismo que el sexo oral.
